IVF and ICSI are the most prevalent fertility treatments, in which sperm and eggs are fertilised in the lab, outside of the mother’s womb, and the embryo is transplanted to the uterus of the ‘to be mother.’ This post discusses both of these and which is superior.
How and When IVF and ICSI Came to Be
The British nurse Jean Purdy, the physiologist Robert Edwards, and the obstetrician and gynecologist Patrick Steptoe developed the in vitro fertilisation treatment. Gianpiero Palermo, who had set the procedure while a student at the Vrije Universiteit Brussel, was responsible for the first recorded ICSI birth in 1992.
What Is The Distinction Between IVF And ICSI?
The procedure by which sperm fertilises an egg is where the fundamental difference between IVF and ICSI lies.
In IVF:
Since no aid is given to the sperm, egg fertilisation occurs naturally when the egg and semen are left in a petri dish. It is only feasible if the sperm are healthy and motile or can swim to the egg and fertilise it.
With ICSI:
Fertilisation occurs when the embryologist chooses the healthiest sperm from the semen and injects them into the egg. Denudation refers to the process by which the egg sheds a covering of cells to facilitate sperm entrance. It is done when the sperm cannot fertilise on their own. After a careful evaluation, your doctor will explain why both methods are appropriate in your case.
IVF Methodology
There are several stages in the IVF process that together take about three weeks.
- Chemical stimulation of the ovaries: this helps them generate more eggs, mature, avoid early ovulation, and get your uterine lining ready.Your eggs will be ready for collection in about ten days.The doctors will use blood tests and vaginal ultrasounds to determine this.
- Egg Retrieval: Occurs in a medical facility before ovulation, roughly 36 hours after your last hormonal injection.The physicians will give you drugs and put you to sleep to keep you from feeling anything.A vaginally implanted ultrasonography probe locates and identifies follicles, and a thin needle coupled to a suction device removes the eggs from the follicles.The eggs are retrieved using transvaginal ultrasound aspiration. The liquid these eggs are stored in is nutrient-rich.
- Sperm Collection: On the day of egg retrieval, a sperm sample will be taken.To sort out the best quality semen, a high-speed washing cycle is used.
- Fertilisation: The healthy sperm and the mature eggs are placed in the dish with the eggs. The resulting embryo is then given 3-5 days to develop in the lab. This method, known as “standard insemination,” requires an incubation period to ensure conception.
- The Embryo Transfer Procedure: When the time comes, your doctor inserts a catheter into your uterus and transfers the embryo or embryos.
The ICSI Procedure
The first two stages, ovulation stimulation with hormones and egg extraction, are identical to in vitro fertilisation (IVF). Then the ICSI procedure involves:
- Gg Denudation: Removing the cell layer surrounding the mature eggs (known as ‘oocyte denudation’) can be done mechanically or chemically after egg retrieval.
- Selecting Sperm: The best sperm from the collected semen are chosen for further study.
- Fertilisation: ICSI involves injecting the sperm directly into the egg’s cytoplasm. Embryo transfer occurs three to five days after egg retrieval, and the remaining processes of embryo development and embryo transfer are the same. After 14 days, a pregnancy test is taken.
ICSI and IVF For Your Health, Which Is Better?
Both aim to create a healthy embryo, but your fertility issues will determine which method is most effective for you. You may only need IVF in some instances, but in others, you may be advised to try ICSI to help develop an embryo.
It is an excellent idea to do IVF when:
- Regular attempts to conceive have failed.
- The ovaries are affected by cysts.
- There is obstruction or injury to the fallopian tubes.
- Your ovulation cycles are unpredictable.
It is an excellent idea to do ICSI if:
- A failed IVF cycle occurs.
- The sperm was obtained surgically, it may be irregular in shape or have poor movement, contributing to a low sperm count.
- Problems occur with getting an erection or ejaculating.
- There are many antibodies in the sperm.
